Rescheduling / Cancellations. In the event you need to reschedule, or cancel your appointment,. kindly give 36 hrs notice. You can do this by calling or emailing me. Your appointment time is a contract between us. If you cancel without sufficient notice, you will be charged in full.
Please be advised , that we do not offer the advice or services of physicians, psychologists, or therapists. However, we do have a referral list of professionals in those areas if needed.
Insurance
We are unable to accept insurance, but we will be glad to provide you with a receipt that can be submitted to your insurance company to cover your hypnosis. Please ask your insurance company, if there is any treatment or diagnosis codes, that may have to be included on your receipt. Coaching is not usually covered by insurance.
Information Form
This is a form that you can download, fill out, and bring it with you to your first appointment. It is a brief form for your name, address, why you chose hypnosis etc. Or, you can fill it out when you arrive at your appointment.
Payment
I accept major credit cards, MasterCard, American Express, and Visa, and personal checks, as well as cash.
Office Locations:
I have office locations in Palatine, Highland Park and Chicago. I can also make on-site visits (hospitals before and after treatments and surgery, and businesses when appropriate). My evening and week-end appointments usually book up quickly, so if that’s the best appointment time for you, please schedule early.
